This 3-week intensive leisure education program, facilitated by Recreation Therapists Faith Jasper (CTRS) and Courtney Addison (CTRS), is designed to support individuals within Transitional Care Units as they prepare for discharge into the community. Originally presented in 2022, the program has since been refined based on participant feedback, outcome tracking, and interdisciplinary collaboration. The program focuses on helping patients/clients rediscover meaningful leisure interests, explore accessible community resources, and develop the confidence and skills necessary to engage in leisure as a component of a healthy lifestyle post-discharge. Over the course of three weeks, patients participate in structured group sessions targeting leisure awareness, self-efficacy, realistic problem solving, coping strategies, and community reintegration planning. This updated presentation highlights key findings, observed benefits, and adjustments made to enhance patient engagement and relevance. Interactive components mirror actual interventions used in sessions, offering attendees a hands-on look at the techniques used to motivate, educate, and empower patients during their transition from care to community living. The content of this program highlights the importance of Recreation Therapist’s role within Transitional Care as well as the discharge process and potential to grow the profession in the community direction to alleviate healthcare burden and readmission. .

Social Prescribing in Manitoba - What is it and what does it mean. Social prescribing: brings community capacity and healthcare services more closely together, to connect people to community-based programs or services that can improve their social connections, health and well-being. Connie Newman, Executive Director (Manitoba Association of Senior Communities - MASC) will present. MASC connects with 88 senior groups across Manitoba, as well as 97 municipalities along with the Social Prescribing Initiative.
